#44: Implement dependency injection in MigrationScriptExecutor

- Created IMigrationExecutorDependencies interface for optional service injection
- Updated MigrationScriptExecutor constructor: (handler, logger?) → (handler, dependencies?)
- Changed service properties to public readonly for testability
- All services use nullish coalescing (??) for defaults
- Added 6 comprehensive tests for dependency injection (191 total tests)
- Updated documentation: API reference, migration guide, custom logging guide
