feat: simplify TLS testing and fix compatibility issues

- Add tls-rustls feature to sqlx for TLS support
- Fix DSN format: use @tcp(host:port) instead of @host:port
- Fix MariaDB 12+ compatibility: handle @@read_only as BIGINT
- Simplify justfile: one command 'just test' runs everything
- Remove docker-compose dependency: use pure podman
- Remove obsolete scripts: setup-test-dbs.sh, setup-tls-tests.sh, docker-compose-tls.yml
- Add TLS_TESTING.md quick reference guide
- Update all documentation for simplified workflow

All 73 tests passing (37 unit + 21 integration + 15 TLS)
