Extract bash parser helpers and ban command chaining

Extracted extractCommandNames into bash-parser-helpers.ts for reusability.
Added hasChainOperators function to detect &&, ||, and ; operators using
shell-quote parser to avoid false positives from operators in strings or
comments. Updated pre-tool-use hook to ban bash command chaining for better
tracking and error handling. Renamed test file to match new helpers module
and added comprehensive tests for hasChainOperators.
