refactor: Rename decode_sse_event to decode_stream_event for protocol agnosticity (#166)

Renames the Provider callback from decode_sse_event to decode_stream_event
to reflect that providers use different streaming protocols:

- OpenAI, Anthropic, Google: Server-Sent Events (SSE)
- AWS Bedrock: AWS EventStream (binary)

The old name was misleading for non-SSE implementations like Bedrock.

Changes:
- Updated Provider behavior definition with clearer documentation
- Renamed all provider implementations (Anthropic, OpenAI, Google, Groq, Bedrock)
- Updated all tests and test helpers
- Updated guides (adding_a_provider.md, core-concepts.md)
