chore: Extract ErrorFallback to external lib

Migrates the custom ErrorFallback component and the stack trace
decoding logic (`useReadableStack`) into the new external package
`@rm-hull/chakra-error-fallback`.

This extraction significantly simplifies the application structure.

This commit removes:
* `src/components/ErrorFallback.tsx`
* `src/hooks/useReadableStack.ts`
* `source-map-js` dependency

Also updates the style of the loading progress bar in `Loader.tsx`.
