refactor: Extract core logic into app package

Move the main application execution flow (git diff, LLM call, user
interaction, commit) from `main.go` into a new `internal/app` package.

This separation improves:
* Maintainability by isolating core application functions.
* Testability of the main execution loop.

Includes minor style fixes in LLM providers and git diff
exclusions.
