Always specify host name for ClickHouse registry

Default to "localhost" if it's not set to align with the behavior of the
ClickHouse client, and to avoid URI::db::_odbc returning
`dbi:ODBC:DSN=sqitch;Driver=ClickHouse`, which won't exist.

Also test ClickHouse 25.9 and remove irrelevant pod.
