enhance(config): Support `enable` field in tool defaults (#302)

Users can now configure a default enabled/disabled state for all tools
through the `tools.defaults.enable` setting. This provides a convenient
way to control tool availability globally while still allowing per-tool
overrides.

When determining if a tool is enabled, the configuration system now
checks the tool-specific enable setting first, then falls back to the
`tools.defaults.enable` setting, and finally defaults to true if neither
is configured. This maintains backward compatibility while adding more
flexibility to tool configuration.
