* Improve the support for optional headers

Previously, the user would have to `#include
"qiskit/addon/sqd/support/boost_dynamic_bitset.hpp"` _before_ including
`fermion.hpp`, which was brittle and fragile.  This uses `__has_include`
to be much more robust.

* Forgot to add a new file
