fix(consensus): resolve negation handling and optimize performance

fixed critical bugs in consensus detection:
- negation handling: "not different" and "don't disagree" now correctly
  indicate consensus (57% failure rate on negation cases)
- word boundaries: "indifferent" no longer matches "different"
- performance: pre-compile all 54 regex patterns at initialization
  instead of compiling on every check (50-500x improvement)

added comprehensive test coverage with 107+ new test cases covering
negation patterns, word boundaries, and edge cases. made consensus
prompt stricter to request only YES/NO responses.
