Add comprehensive documentation on monitoring Raft cluster health and
performing maintenance operations.

- Add monitoring-maintenance.md covering:
  - Using metrics API to monitor cluster health

  - When and how to add/remove nodes
  - Safety guidelines for automated maintenance
  - Example automated node replacement implementation

- Refine dynamic-membership.md:
