refactor: restructure project to app-based layout

restructure project from cmd/internal layout to app/ based structure
for cleaner organization. update all imports and configuration files
to reflect new paths.

changes:
- move cmd/local-docs-mcp -> app (main entry point)
- move internal/{scanner,server,tools} -> app/{scanner,server,tools}
- update imports across all packages
- improve main.go with slog, realMain pattern, signal handling
- fix help flag check to use direct enum comparison instead of errors.Is
- update ci/cd configs, makefile, and documentation paths
- remove redundant main_unit_test.go
