Improve Dockerfile (#38)

* Improve Dockerfile

Updates to improve the docker image and build:

* Use official elixir image as base to simplify updating
* Add a dockerignore and update RUN statements to enable caching of build layers
* Move entrypoint into dedicated file
* Add VOLUME and EXPOSE statements
* Move checking secret key base into elixir config with dedicated file env var
  to eliminate bash dependency and enable docker secrets

* Update docker-compose and README
