refactor(llm, google): Move instructions and attachments to system instruction (#296)

Updates `gemini_client_rs` to `0.6.2` and adapts to API changes.
Instructions and attachments are now included in the system instruction
block alongside the system prompt, rather than being injected into the
conversation history with a fake assistant acknowledgement.

This results in cleaner conversation history and better aligns with how
system-level context should be provided to the model.
