Add hex.pm best practices: changelog link and module grouping (#120)

Improves hex.pm package quality by:
- Adding changelog link to package metadata pointing to hexdocs
- Including CHANGELOG.md in docs extras and package files
- Organizing documentation with module groups (Providers, Steps, Streaming, Data Structures, Provider API, Core)
- Adding changelog section in docs extras groups

These changes follow hex.pm best practices and improve documentation discoverability.
