test: achieve 100% test coverage for all source files

- Add coverage filtering script to exclude mocks, generated files, and test utilities
- Add SigningTest for SigningFacet.generateSignaturePayload()
- Add LibGovTokenTest with helper facet to test LibGovToken transfer functions
- Add test for InitDiamond reinitialization error path via diamondCut
- Update test-cov script to run coverage filter after tests
