Additional dependency updates and refactoring

* Kamon 0.6-related updates:
    * Prometheus extension will not auto-start
    * PrometheusEndpoint:
        * Make snapshot package-private
        * Do not pass snapshot value in as constructor parameter
    * PrometheusExtension:
        * Remove `flushBuffer`, no longer needed
        * Remove `requestSnapshot`, defer this until the library is broken up
        * Use configuration from Kamon
        * Check tick interval vs refresh interval here
    * PrometheusSettings:
        * No longer check against Kamon's tick internval here
    * Testing has been simplified:
        * KamonTestKit
             * Now just an object with utility methods
             * No longer attempts to modifiy config in Kamon
             * Contains a simpler way to clear all entities from Kamon
        * PrometheusSettingsSpec
             * Now only tests things that are independent of Kamon
             * Tests that require different configurations are now in custom SBT test configurations
        * PrometheusExtensionSpec and SnapshotConverterSpec uses fixtures to deal with Kamon global state
* Dependency updates:
    * Akka 2.4.6 → 2.4.9
    * Scala 2.11.7 → 2.11.8
    * ScalaCheck 1.12.5 → 1.13.2
