fix(duplicate): run duplicate detection for approved users

approved users were bypassing duplicate detection due to pre-approval
check returning early. duplicate detection is a behavioral check that
should run for all users, not just unapproved ones.

changes:
- move duplicate detector check before pre-approval check
- pre-approval now only skips if no spam detected by behavioral checks
- approved users still skip expensive content checks (similarity, classifier)
- add comprehensive tests for approved user duplicate detection
- update README to document behavioral vs content check distinction

this ensures approved users cannot spam duplicates while maintaining
performance optimization for trusted users.
