Additional #empty? Fixes and Documentation Warning

`Aws::DynamoDB::Client#scan` can return empty pages, so the `#empty?`
implementation needed some additional checks.

Also added documentation to these methods, including stern warnings
about the risks of calling these methods, given that they can easily
result in full table scans, or large memory allocations.
