enhance: Improve empty response retry by appending context to query (#274)

When an empty response is received during query processing, the retry
mechanism now appends a message to the original query informing the LLM
that the response was empty and asking it to try again. This makes the
retry attempt more likely to succeed by providing context about why the
retry is happening, rather than blindly repeating the same query.

The system appends "The response appears to be empty. Please try again."
to the query on first retry, and "Please try again." on subsequent
retries if the message is already present.
