feat: add support for multiple custom OpenAI-compatible providers

- Add new --customs flag for configuring multiple custom providers using compact ID:key=value format
- Support environment variables with CUSTOM_<ID>_<FIELD> pattern for each provider
- Maintain backward compatibility with legacy --custom.* flags
- Implement proper precedence: CLI > Legacy > Environment
- Add comprehensive validation for provider IDs (only [a-z0-9-_] allowed)
- Move all custom provider logic to dedicated custom_providers.go file
- Add extensive test coverage including unit and integration tests
- Update README with detailed documentation and examples

This allows users to configure multiple custom providers like OpenRouter,
local LLMs, and other OpenAI-compatible services simultaneously.
