Implement refresh parameter as part of cache key instead of cache bypass

- Modified CacheRouter.getZerionPositionsCacheDir to include refresh parameter in cache field
- Updated ZerionPositionsApi to pass refresh parameter and removed cache-skipping logic
- Cache field now includes refresh value: "USD_false" vs "USD_true"
- This creates separate cache entries for normal vs refresh requests instead of bypassing cache
