Enforce structured JSON logging in Kubernetes environments (#1775)

* Enforce structured JSON logging in Kubernetes environments

- Add UNSTRUCTURED_LOGS=false to operator deployment via Helm template
- Automatically set UNSTRUCTURED_LOGS=false for all proxy runner containers
- Add test coverage to verify structured logging is enforced
- Ensure setting cannot be overridden by users

This ensures all components in Kubernetes use structured JSON logging,
which is the best practice for cloud-native applications and improves
observability through better log parsing and aggregation.

* bump versions and run helm-docs
