config: add dns refresh rate options (#5819)

## Summary
Add `dns_failure_refresh_rate` and `dns_refresh_rate`. These customize
the rate at which domain names are refreshed in envoy.

Also
- rename `getDNSCluster` to `GetDNSCluster` and add unit tests for it
- rename `dns.go` to `options_dns.go` to be consistent with databroker
options
- move from/to proto logic for `DNSOptions` to methods
- rename `copyOptionalDuration` to `copyDuration` and add a new
`copyOptionalDuration` that takes a pointer to a duration
- add unit tests for the dns options
