Improving the Readme (#524)

* ...in order to improve clarity and focus. When I look at a readme, I want to know "When should I use this module?", "How does this module compare to its competitors?", "How do I get started?", and "What are the most common configuration deviations and friction points?".
* ...in order to improve formatting, group related content, and promote the readme's table of contents.
* Updated Additional Reading to include all docs. Looks like there's some near duplicates that could be removed or merged.
* renamed documentation heading to TOC
