Use date_uploaded instead of date_created

This change introduces three enhancements:
* Using date_uploaded because date_created is sometimes modified
  to flag items that have had major changes
* Adding an adjustable starting date
* Adding the option to report in yearly, quarterly, or monthly ranges

This change *does not* include UI components for selecting the start
date or reporting period.
