Refactor Olivine index management with cleaner separation of concerns

- Extract transaction processing logic into IndexUpdate module
- Simplify Index struct by removing transient tracking fields
- Rename methods for consistency (fetch_page_for_key → page_for_key)
- Streamline transaction application with better state isolation
