refactor: streamline timeout middleware configuration and enhance error handling

Updated the timeout middleware to enforce a required `ttl` parameter and removed the optional custom error message, simplifying the configuration. Enhanced error handling by implementing an AbortController for better management of timeout scenarios. Adjusted related tests and added the timeout middleware to the global middleware exports.
