Fix all failing tests - complete test isolation

- Added beforeAll/afterAll hooks to save/restore environment variables
- Improved mock cleanup with jest.clearAllMocks()
- Fixed mock storage reset to use proper defaults
- Added PROXY_HOST to environment cleanup
- All 68 tests now pass successfully

The issue was test pollution between test files when mocks were not
properly isolated. Tests now pass both individually and when run together.
