refactor: Add new type `UtxoTriple`

The new type represents the preimage to transaction output, consisting
of the UTXO, the sender randomness, and the receiver digest. This tuple
occurs in many places throughout the code base; now it has a name.

Also: avoid calling `mutator_set::commit` directory to produce an
`AdditionRecord`. Whenever possible, produce a `UtxoTriple` object and
use its convenience function `addition_record()` instead.
