docs: expand error codes documentation with reasons and debugging steps (#1006)

Expanded the error codes documentation in docs/error_codes.md. For each
error code, added detailed explanations of why the error occurs and how
to debug or solve it, based strictly on the actual codebase and tests.
This improves developer experience and helps users quickly identify and
resolve issues related to Safe Smart Account error codes.
Fixes #386
