Add gRPC ContextManager implementation. (#595)

This propagates (and reactivates) ContextSnapshots _over_ gRPC Context,
but _also_ propagates gRPC Context _in_ captured ContextSnapshots.

So either method (gRPC Context or ContextSnapshot) will propagate both
types of context.
