Docker dev env improvements (#101)

Improvements:
- Better health check configuration
- Fix for when `manage.py` is not executable
- `docker_setup.sh` script to run migrations and create debug users (can
hopefully be improved in the future)

Docs have also been improved and updated to reflect this change.
