Improved, fixed and moved installation docs

Details:

* Created a new main chapter "Installation" in a new file installation.rst,
  for keeping all installation documentation.

* Added a section "Steps" in the new "Installation" chapter, in order to
  show how the sections in that chapter are used together.

* Moved the sections for the various installation approaches and for HMC
  setup and proxy/firewall setup into the new "Installation" chapter.
  Minor improvements in these existing installation sections.

* In section "Setting up the HMC", added items for configuring a CA-verifiable
  HMC certificate, and for enabling the WS-API flag for the HMC userid.

* In section "Installation on a system without Internet access", added that
  the download and target system either need to have compatible platform,
  Python version and implementation, or that these parameters need to be set
  with pip options.

* Renamed section "HMC certificate" to "Using HMC certificates" in order
  to clarify that this is not about the setup of HMC certificates, but about
  using them with the zhmc command.

* Moved the section about running in a Docker container from the "Introduction"
  chapter into the "Command line interface" chapter.
