Refactor provider initialization for better modularity

- Add initializeProvider function to replace createStandardProvider
- Refactor initializeProviders to use configuration-driven approach
- Update initializeCustomProvider to return values instead of using pointer parameters
- Update tests to match new function signatures
- Pre-allocate slices for better performance
