Update README.md to reflect changes in default model and environment variables

* Change default model to Phind 70B
* Add missing environment variables: `OPENROUTER_API_KEY` and `COPILOT_TOKEN`
* Add instructions for obtaining COPILOT_TOKEN by configuring the Copilot extension in VSCode and extracting the token from `~/.config/github-copilot/apps.json`
