* Add smart exclusions with .gitignore and common patterns support

- Add automatic exclusion of common ignored directories/files
- Add support for .gitignore pattern integration
- Add safeguards and limits for .gitignore file processing
- Add warning for unsupported .gitignore features (negation)
- Improve pattern handling with deduplication and pre-allocation
- Update documentation with smart exclusions feature details

* Fix CI tests by removing tmp exclusion pattern

- Remove /tmp exclusion from common exclusion patterns to prevent test failures
- Update TestCommonIgnorePatterns to verify this behavior
- Update README documentation to reflect the change
