Add configurable file size limit and JSON output format

- Add a new --max-file-size flag (default: 64KB) to control the maximum size of files processed
- Update all file loading functions to accept and respect the file size parameter
- Update tests to use a larger max file size for test files
- Add documentation for the max file size option in README.md
