Refactor code structure and improve error handling (#13)

* Refactor code structure and improve error handling

- Created prompt builder package to separate file loading from main logic
- Added provider factory for simplified provider creation
- Enhanced error handling in runner with more detailed messages
- Improved documentation for better code maintainability

* Improve error handling for failed providers

- Log provider errors instead of showing them in the output
- Show only successful provider responses to users
- Update tests to match new behavior
