* fix(a11y): keyboard focus for dialogs

A quick patch for broken keyboard focus in the `Modal` component. Manually focus the component root when it mounts.

Also, add the `dialog` role along with an accessible name for the dialog (using `aria-label`.)

* fix duplicate roles and accNames in DisabledTaskPopup

* allow parent components to turn off autofocus behaviour

* make autofocus opt-in, not opt-out

* add trapFocus to SubTaskPopup and ConsensusPopup

* move dialog role and trapFocus to withLayer
