Add pluggable formatters for traceable matrices

This commit contains the following changes:
- Re-factor the matrix processor to use flexible formatting
  classes, similar to those introduced for traceable displays
  in commit c651c87.
- Introduce FormatProcessorBase as a common base class for node
  processors that support specifying which formatter to use to
  generate output. Currently the traceable displays and traceable
  matrices use this.
- Standardize all custom nodes to use attributes with "traceables-"
  prefix.
- Standardize all custom nodes to use attributes "source" and "line"
  to store where in the source files they originated.
