Extract the notify API in order to expose a manual `notify` endpoint (#34)

In https://docs.bugsnag.com/platforms/writing-a-notifier/, features, "Capturing handled errors", just applying points 1 and 2 (3 wouldn't be a low-hanging fruit so we might just do that when we need it).

The change is mostly about moving the internal code from the logger handler into a publicly exposed endpoint.
