Coveralls, 3.3/3.4 AppVeyor, modernize, pinning.

Modernizing helper files. Using the same ones from my other projects.
Adding a CONTRIBUTING.md file like my other projects.

Switching from codecov to coveralls.

Testing 3.3 and 3.4 on AppVeyor.

PyPI no longer keeps track of download counters, removing from README.

Using pylint in tests. Moved two variables down in setup.py to fix
linting.

Pinning test/lint versions to prevent surprise broken tests.
