fix: update target-title help and fix exclude-pr default

- Update help message for --target-title to list available fields
- Change default ExcludePR value from false to true
- Update cmd document

This commit improves the documentation for the --target-title flag and fixes the default behavior to properly exclude pull requests from planning content by default.
