update approved users storage with multi-db support

- Simplify initial schema checks to be database-agnostic and uniform across tests.
- Update schema and index creation queries to support multiple databases (SQLite & Postgres).
- Consolidate migration logic within the ApprovedUsers struct, handling table and index creation via transactions.
