Use exception structs; add raising persistence functions

There was a pattern to match against `:ok` everywhere which
would lead to match errors obscuring the file I/O errors causing them.
This adds raising versions of the persistence versions so that all
of those matches aren't needed.

This had a side effect of requiring consistent use of exception structs
so that bare atoms weren't being raised. This made it possible to
tighten specs.

BREAKING CHANGE: snapshot functions now raise if persistence isn't
enabled. They used to return `:noop`, but this forced more `:ok = `
matching to satisfy Dialyzer. Since this feels more like the programmer
made a mistake about not enabling persistence, `:noop` is now an error.
