Support non-contiguous selection (#43)

`range_reference::to_string`: do not use range notation for a single
cell similar to other ECMA 376 implementations. Note that
`xlsx_producer` performed this logic itself, resulting in code
duplication.

`test_defined_names`: fix unit test as `to_string` doesn't use a range
notation anymore for a single cell. Note that also the provided xlsx
file (19_defined_names.xlsx) stores this value as "$B$4" instead of
"$B$4:$B$4".
