[MAINTENANCE] Remove dead code - EtdForm committee support

**RATIONALE**
The logic for committe chair and members has been re-designed and
moved to the JavaScript UI. The code here reference obsolete
affilitation names and no-longer used affiliation_type parameters.

This change removes obsolete methods to simplify the form model.
