Pin dependency versions for reproducible installs  (#13112)

* Add constraints for core dependencies

This commit adds constraints to pin library versions using the
pip-compile tool. This was generated on Linux without the --all-extras
parameter

* Add release note about constaints.txt

* Add extras

Using the --all-extras parameter when calling to pip-compile, now that
qiskit-qasm3-import is constrained to v0.3.0.
Also bumping version for 0.64.3 in this branch.

* Add Windows constraints

* Update constraints for MacOS

* Fix numpy version for Python>=3.12

* Pin numpy to 2.0.0 for Python 3.12

Numpy version 1.26 causes CI failures in test_isometry.py on MacOS

* Add constraints.txt to MANIFEST.in
