Update data dictionary variable class and change ordinal naming to nominal (#108)

- Include all MVP class variable in data dictionary
- Update database to use intermediary table instead of view to optimize for speed
- Data Dictionary now no longer depend on 'catch all' cohort
- Change naming from ordinal to nominal
